As StinkyJournalism wrote in late September, UK news station ITV News apologized after it aired fake images from a video game in its news program focusing on "the relationship between" Muammar Gaddafi and the Irish Republican Army.  Instead o showing footage of "the IRA supposedly shooting down a helicopter with weapons supplied by Muammar Gaddafi" in 1988, the network showed the video game.

Now, UK regulator Ofcom has announced it is looking into the error.  OfCom explained its investigation is to determine if "the incident constituted a breach of its broadcasting code," according to the Guardian.

Journalism.co.uk noted that as of last month's apology, at least 25 complaints were filed with Ofcom over the error.
